Service Description
Initial medical consultation for weight management. This visit includes a comprehensive health history review, medication screening, lifestyle discussion, and determination of eligibility for medical or wellness-based weight management programs. Treatment options, expectations, and next steps will be reviewed. Enrollment in a program is required prior to scheduling.
Cancellation Policy
Appointments must be scheduled through the official booking system and are based on provider availability and program eligibility. Clients may cancel appointments online without penalty more than twenty-four (24) hours prior to the scheduled start time. Appointments cancelled within twenty-four (24) hours of the appointment time or missed without notice may be charged a fifty-dollar ($50) late cancellation or no-show fee to the payment method on file. Clients may reschedule appointments online up to twenty-four (24) hours prior to the scheduled start time. A valid payment method is required to be kept on file at the time of booking to secure appointment reservations and any applicable cancellation or no-show charges. Placement on a waitlist does not guarantee appointment availability. If an appointment becomes available, clients may book based on real-time availability. Appointment access may be limited based on membership status, program enrollment, or clinical eligibility. Booking an appointment does not guarantee medical clearance for treatment. These policies are provided in accordance with Connecticut consumer protection standards and are subject to update with reasonable notice.
